All claims must be submitted within 14 days from the occurrence of the incident, unless otherwise stated. Please complete and mail the original completed Claim Form (download) together with the relevant supporting documents (refer below):

The list of documents stated above is not exhaustive. All medical reports and information required by us shall be furnished at the policyholder or claimant expense. Other documents may be requested if necessary, which we will advise you in writing.
